Name that UI/Add-on!

Can someone tell me what UI is being used in this video, as well as the add-on or if its part of the UI shown.

I'm still unsure as to what these are (circled in red in this picture: http://i25.tinypic.com/2r6dzps.jpg). They seem to be cooldown/action bars for skills. Any idea what addon-on this is?

I'm also interested in finding out what this is called and the actual addon for it. (http://i28.tinypic.com/1zyyiwy.jpg) It appears to be a 3d image of your current target. (what led me tot think what being used was more than a standard UI).

If anyone knows the names of these HUD, SCT, Combat text mod, UI, and action bar mods. I would greatly appreciate it. XD|||The 3d model thingy is probably something like ZMobDB (i think that's what its called)
